Job summary

Qualifications

• RN license active in the state where you work or eligible
• 3 years nursing/clinical case management experience preferred
• BSN degree preferred but not required
• Knowledge of CMS guidelines and utilization management processes

Responsibilities

• Conducts complete assessments and develops care plans within timeframes.
• Coordinates discharge planning and transition planning with patient/family and team.
• Provides case management across health-care levels including housing, finances, and access to services.
• Initiates referrals and documents utilization reviews in a timely manner.
• Collaborates with MDs, social workers, and care teams to resolve barriers to discharge.
