University Health seeks a Nurse Case Manager to coordinate the treatment plan for patients within the acute episode of care. You will collaborate with patients and an interdisciplinary team to ensure timely delivery of services, evaluate interventions, and communicate health care needs to advocates and providers.

Responsibilities include tracking variances in the plan of care, promoting patient advocacy, and facilitating efficient care delivery across care settings.

Qualifications

• Bachelor’s degree in Nursing preferred.
• Three to five years nursing experience required (Staff nurse II or above).
• Experience in case management, utilization review or hospital quality preferred.

Responsabilités

• Coordinate treatment plan for patients within the acute episode of care.
• Collaborate with the patient and interdisciplinary team to assure timely delivery of services.
• Advocate for patient needs and communicate health care requirements to providers and caregivers.
• Evaluate effectiveness of interventions and track variances in the plan of care.
