Job summary

Ascension is seeking a Registered Nurse for Case Management to coordinate the overall interdisciplinary plan of care from admission to discharge. You will advocate for resources, monitor medical necessity, and collaborate with the care transition team to ensure effective care plans and post-discharge services.

The role requires Wisconsin RN licensure, BLS, and CCMC credentialing preferred; a nursing diploma is required.

Qualifications

• Registered Nurse credentialed from the Wisconsin Board of Nursing prior to hire date.
• Licensure from the Wisconsin Board of Nursing OR current home state license if multi-state/Compact.
• BLS Provider (American Heart Association or American Red Cross) preferred.
• Case Manager credential (CCMC) preferred.
• Diploma from an accredited nursing program or equivalent licensure at time of hire.

Responsibilities

• Coordinates plan of care across admission to discharge for Case Management.
• Monitors medical necessity and level of care through assessments and records.
• Maintains dialogue with supervisor and care transition team for plan reevaluation.
• Acts as liaison for documentation and post-discharge services; collaborates with payers for benefit optimization.
