Job summary

PACS is seeking a qualified nursing professional to coordinate post-acute care, communicating plans with residents and families while guiding the care team. Strong clinical experience and familiarity with Medicare/Medicaid are essential, with an emphasis on timely documentation and coordination of admissions and discharges.

The role requires collaboration with physicians, nurses and support staff to ensure safe, cost-effective care in a dynamic, high-priority setting.

Qualifications

  • Bachelor’s degree in Nursing or Social Work required.
  • Registered Nurse license preferred; LVN/LPN acceptable.
  • Minimum two years of clinical nursing experience preferred.
  • Knowledge of Medicare, Medicaid programs and benefits.
  • Ability to interpret procedures and policy manuals.

Responsibilities

  • Communicate regularly with residents and families about plans of care and treatment.
  • Attend morning meetings to facilitate team communication.
  • Monitor and adjust residents' statuses based on changing needs.
  • Complete documentation and perform utilization review activities.
  • Coordinate admissions, discharges and referrals with care teams and providers.
  • Maintain professional interactions with residents, families, staff and physicians.

Education

Bachelor’s Degree in Nursing or Social Work
RN license
LVN/LPN license
CCM certificate
