Job summary

WellMed in South Austin is seeking an RN Case Manager to support high-risk patients in navigating the healthcare system. You will educate, advocate, and collaborate with a multidisciplinary team to improve quality of life.

Responsibilities include identifying high-risk patients, chronic care management, transitions from inpatient to outpatient care, and accurate documentation in the EMR. Requires RN licensure, BLS, and 2+ years in a clinical setting; bilingual English/Spanish preferred.

Qualifications

• RN with active, unrestricted license in state of employment
• Bachelor’s or Associate’s degree in Nursing with 4+ years of RN experience
• Current BLS certification
• 2+ years of clinical office/hospital experience
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD)
• Proficient with electronic medical records
• Strong organizational and prioritization skills
• Good interpersonal skills and teamwork
• TB screening and immunity documentation required

Responsibilities

• Identify high-risk patients with providers and clinic staff
• Support longitudinal care, including assessments, med reconciliation, and patient education
• Assist with transitions from inpatient to outpatient care
• Coordinate with providers to update plan of care
• Document accurately in EMR
• Attend PCC meetings and weekly reports
• Maintain current nursing competencies and knowledge of best practices
• Perform other duties as assigned
