Central Health is seeking an RN Case Manager for High Risk Populations in Austin to lead multidisciplinary Case Management teams, assess medical and social needs, and coordinate care across services. Qualifications

• Graduation from an accredited School of Nursing.
• Current unrestricted RN license to practice nursing in the State of Texas
• Basic Life Support (BLS) certification through AHA Training Network or Red Cross.

Responsibilities

• Conduct patient assessments, including home visits and community visits to evaluate medical, behavioral health, and social needs.
• Provide rapid clinical assessment and intervention during crises to ensure patient safety.
• Develop and manage culturally responsive, evidence-based care plans with measurable goals.
• Coordinate care across primary care, specialty, dental, behavioral health, and community systems.
• Complete hospital referral reviews, discharge planning, medication reconciliation, and timely appointment coordination.
• Support medication management and patient education for PCP prescribed therapies.
