Job summary
WellMed, part of the Optum family, is seeking a Healthcare Manager to support patients with high-risk conditions and navigate the healthcare system. You will educate, empower, and advocate for patients and families, collaborating with physicians, nurses, pharmacists, and social workers.
The role emphasizes self-directed nursing tasks, care coordination, and comprehensive documentation in the EMR to drive improved health outcomes.
Qualifications
- RN license in state of employment
- BLS certification
- Experience in physician's office or hospital setting
- Knowledge of chronic diseases such as COPD/asthma, diabetes, CHF, IHD
- Strong documentation in EMR
- Excellent verbal and written communication
- Ability to interact with multidisciplinary teams
- Organizational and prioritization skills
- TB screening and immunity proof requirements
Responsibilities
- Identify high-risk patients and support longitudinal care
- Conduct assessments and medication reconciliation
- Provide patient education and refer to appropriate resources
- Assist with care transitions from inpatient to outpatient settings
- Coordinate with providers to update care plans
- Document in EMR and participate in PCC meetings
- Prepare reports for weekly meetings
- Maintain current nursing competencies
