Qualifications

• RN license active and unrestricted in the state of employment.
• Bachelor's or Associate degree in Nursing required with 4+ years of RN experience.
• Current BLS certification is required.
• 2+ years in a physician’s office, clinical, or hospital setting.
• Knowledge of chronic diseases (COPD/diabetes/CHF/IHD) is preferred.
• Proficient with electronic medical records systems.
• Fluency in English; Spanish language skills are preferred.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care: assess conditions, reconcile meds, set self-management goals, educate patients.
• Coordinate transition from inpatient to outpatient care, assess needs and contingency plans.
• Document accurately in the EMR and support PCC meetings and weekly reports.
• Maintain current nursing competencies and participate in related duties as assigned.
