Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager to support patients with high-risk conditions and navigate the healthcare system. You will educate, advocate, and collaborate with multidisciplinary teams to optimize quality of life and care transitions.

The role involves documenting in EMR, leading care coordination activities, and ongoing professional development in a dynamic setting. Strong RN background and bilingual English/Spanish skills are a plus.

Qualifications

Bachelor's degree in Nursing or Associate's degree in Nursing with four or more years of RN experience.
Active and unrestricted RN license in the state of employment.
Current BLS certification.
2+ years of experience in a physician's office, clinical or hospital setting.
Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD.

Responsibilities

Identify patients at high risk with providers and clinic staff.
Support longitudinal care: assess conditions, reconcile medications, motivate self-management goals, educate patients, refer to resources.
Support transition from inpatient to outpatient care: assess needs, reconcile meds, establish contingency plans, educate patients, arrange post-discharge needs.
Coordinate with providers to develop or update plan of care.
Document accurately in the electronic medical record.
Participate in daily huddles and PCC meetings and prepare weekly reports.
Maintain competence and knowledge of evidenced-based practices.
Perform clinical tasks within scope of practice.
