Job summary

Central Health seeks an experienced RN Case Manager to lead multidisciplinary case management for high-risk populations in the Austin area. The role involves conducting comprehensive assessments, home visits, and coordinating with social workers, physicians and community resources to ensure timely care and safe transitions.

You will develop individualized care plans, support process improvements, and advocate for patients navigating housing, transportation and health services.

Qualifications

• Graduation from an accredited School of Nursing.
• RN license to practice in Texas (unrestricted).
• BLS certification from a recognized program.

Responsibilities

• Lead CM team in assessing medical and social needs.
• Perform home visits and referrals to shelters and navigation centers.
• Develop culturally responsive care plans with measurable goals.
• Coordinate care across primary, specialty, behavioral health and community services.
• Ensure discharge planning, medication reconciliation and timely appointments.
