Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager (RN) to support high-risk patients and help them navigate the healthcare system in Texas.

You will collaborate with physicians, nurses, pharmacists and other staff, perform assessments, medication reconciliation, Motivational Interviewing, and transitions from inpatient to outpatient care. An active RN license and BLS are required, with 2+ years in healthcare settings.

Qualifications

• Bachelor's or Associate degree in Nursing with 4+ years of RN experience
• Active, unrestricted RN license in the state of employment
• Current BLS certification
• 2+ years in a physician's office, clinical, or hospital setting
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD)
• Proficient EMR skills to document patient care
• Excellent verbal and written communication
• Strong interpersonal and team collaboration skills
• Ability to interact productively with individuals and multidisciplinary teams
• Strong organizational and prioritization skills
• Tuberculosis screening and immunity proof or vaccination documentation

Responsibilities

• Identify high-risk patients with providers and clinic staff
• Support longitudinal care and chronic disease management
• Perform medication reconciliation and patient education
• Coordinate transitions from inpatient to outpatient care
• Update and monitor care plans with providers
• Document accurately in the EMR
• Participate in daily huddles and PCC meetings
• Produce timely reports for weekly meetings
• Maintain nursing practice competence and evidence-based knowledge
• Perform other related duties as assigned
