Responsibilities

• Coordinate patient care across the continuum to ensure appropriate level of care based on medical necessity.
• Assess transition needs to promote timely throughput and safe discharge.
• Promote prevention of avoidable readmissions through effective utilization and transition management.
• Provide education to physicians, patients, families and caregivers as part of care planning.
• Ensure compliance with regulatory requirements and Tenet policies while maintaining care quality.
