Job summary

Qualifications

• Three to five years of acute care nursing experience or one to three years experience in utilization review and/or discharge planning preferred.
• Bachelor of Nursing Degree required; Significant Case Management experience considered in lieu of Bachelor degree.

Responsibilities

• Coordinate patient care across the continuum of care and mobilize resources.
• Document patient plan of care, progress, goals and interventions.
• Participate in patient care conferences and collaborate with medical staff and nursing.
• Monitor utilization of resources and length of stay; communicate issues to the team.
• Identify gaps in care and advocate for appropriate post-discharge services.
