Job summary

Brown University Health is seeking a registered nurse case manager to coordinate patient care from admission to discharge at Rhode Island Hospital. You will work with the medical team to develop and implement individualized discharge plans, identify high-risk patients, and facilitate timely transitions across the continuum of care.

This role requires Rhode Island RN licensure, a Bachelor's degree in nursing, and a strong foundation in case management and CMS regulations.

Qualifications

• RN licensure in Rhode Island or eligible for licensure.
• Bachelor's degree in Nursing required.
• Case management certification preferred.
• AHA BLS Provider required.

Responsibilities

• Collaborate with medical staff to manage care plans across the continuum.
• Develop and review discharge plans with patient and family.
• Identify high-risk patients and coordinate interventions within one business day of admission.
• Communicate discharge plan and follow CMS/DOH guidelines.
• Document care plans and ensure safe handovers.
• Participate in daily rounds and quality improvement activities.
