Job summary

Brown University Health is seeking a Registered Nurse for the Case Management team at Rhode Island Hospital in Providence. The role collaborates with medical staff to facilitate discharge planning, coordinate care across the continuum, and ensure efficient delivery of services across the patient care pathway.

You will participate in daily rounds, identify high-risk patients, develop discharge plans with the team, and communicate plans to patients and families while navigating CMS and DOH

Qualifications

• RN licensure in Rhode Island required.
• Graduate of a School of Nursing with RI license to practice.
• Bachelor's Degree Required.

Responsibilities

• Partners with medical staff to facilitate the plan of care across the continuum.
• Participates in daily care rounds to evaluate discharge planning and updates.
• Proactively builds post-hospital referrals for timely discharge.
• Communicates discharge plans and CMS requirements to patients and families.
• Documents final discharge disposition and supports safe handovers.
• Identifies opportunities to improve patient flow and reduce delays.
