Responsibilities

• Collaborate with medical staff to facilitate the plan of care for defined patient populations across the continuum.
• Participates in daily care rounds to develop and review discharge plans with patients and families.
• Coordinate post-hospital referrals with the Transition Care Coordinator to aid timely discharge.
• Communicate the discharge plan and post facility care to patients, families, and care teams.
• Document final discharge disposition and ensure safe handovers during transfers.
• Identify opportunities to improve patient flow and reduce service delays.
