Job summary

Brown University Health is seeking a registered nurse to coordinate case management and discharge planning for a defined patient population. You will work with the medical staff to develop and implement care plans across the continuum of care.

The role requires collaboration with the healthcare team, strong analytical skills, and experience with discharge planning processes in a hospital setting.

Qualifications

• Licensed Registered Nurse in the State of Rhode Island.
• Bachelor's Degree Required.
• Certification in Case Management by a nationally recognized organization preferred.
• AHA BLS Provider Required.

Responsibilities

• Coordinate discharge planning across the care continuum.
• Identify high‑risk patients and target interventions within one business day of admission.
• Participate in daily rounds to facilitate discharge planning with the healthcare team.
• Develop and review the initial discharge plan with patients and families.
• Advise patients on financial implications of discharge plans.
• Document final discharge disposition and ensure safe handovers.
