Qualifications

• Knowledge of disease management.
• Three to five years of acute care nursing experience or 1–3 years experience in utilization review and/or discharge planning preferred.
• Bachelor of Nursing Degree required. Significant Case Management experience considered in lieu of Bachelor degree. Current MA RN license.

Responsibilities

• Coordinates patient care across the continuum of care and mobilizes internal and external resources to facilitate the case management process.
• Documents patient plan of care according to department guidelines and communicates with care teams.
• Provides nursing assessment within 24-48 hours of admission for clinically oriented services.
• Participates in patient care conferences and collaborates with medical staff while covering for the unit case manager.
• Monitors utilization of resources and communicates issues to the Outcomes Manager.
• Maintains professional relationships with MCO reviewers and external agencies; ensures timely and accurate data reporting.
• Communicates effectively with internal and external customers, respecting cultural diversity and using interpreters when needed.
