Job summary

Qualifications

• Certification in Case Management preferred; BLS required.

Responsibilities

• Partners with medical staff to facilitate the plan of care across the continuum.
• Identify high-risk patients and coordinate discharge planning within one business day of admission.
• Develop and review initial and daily discharge plans with patient/family and team.
• Communicate discharge plans and post-facility referrals to all care team members.
