Qualifications

• Two years of professional experience in a clinically related area.
• Licenses may be required depending on department; current Utah licenses or equivalent.
• Willingness to work rotating shifts including nights, weekends, and holidays in a 24/7 health system.

Responsibilities

• Coordinates case management from entry to post-discharge with care team.
• Identify patients suitable for case management based on criteria.
• Monitor and document quality of care and progress toward discharge goals.
• Facilitate cost-effective outcomes by determining appropriate level of care.
• Discuss funding/insurance issues with clients and providers to optimize utilization.
• Negotiate with third-party payers regarding benefits and eligibility.
• Plan timely discharges and follow-up arrangements.
• Provide oversight of CMS Important Message issuance.
• May set up follow-up appointments and perform post-discharge calls.
• May conduct home/site visits as departmental needs dictate.
