Job summary

Qualifications

• Bachelor of Nursing Degree required. Significant Case Management experience considered in lieu of Bachelor degree.
• Current MA RN license

Responsibilities

• Coordinates patient care across the continuum of care: mobilize resources for the case management process.
• Document patient plan of care and progress according to Case Management guidelines.
• Provide nursing assessment within 24–48 hours of patient admission for services focusing on discharge planning.
• Participate in patient care conferences and collaborate with medical staff to implement the patient plan of care.
• Monitor utilization of resources and communicate issues to unit case manager and reviewers.
• Maintain professional relationships with MCO reviewers and external agencies during discharge planning.
