21st Century Home Health Services in Novato seeks a Registered Nurse Case Manager, Level 2, to oversee patient care plans in the home setting. You will lead two nurses, perform SOCs, evaluations, and discharge planning to prevent readmissions and promote wellness.

You will collaborate with physicians and the care team, educate families, and ensure timely charting and documentation while handling high-acuity cases with compassion and leadership.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Minimum one year of Home Health Case Management experience.

Responsabilités

• Lead the plan of care and oversee two nurses to implement it.
• Conduct Start of Care, Evaluations, Resumption of Care and discharge planning.
• Educate patients and families; manage wound care, medications, and notes.
• Collaborate with physicians and interdisciplinary team for optimal outcomes.
