Job summary
21st Century Home Health Services seeks a part-time Registered Nurse Case Manager for the Santa Rosa area. You will assess patients in their homes, develop care plans, and coordinate with physicians and the care team to deliver high-quality home health services.
Responsibilities include SOC/OASIS assessment, IV infusions, medication reconciliation, and patient education. CA RN license required; BLS/CPR; valid CA driver’s license; flexible weekend availability.
Qualifications
- CA RN license in good standing required.
- BLS/CPR certification required.
- SOC/OASIS experience highly desired.
- IV medication infusions experience preferred.
Responsibilities
- Perform Start of Care (SOC) admissions and OASIS assessments.
- Conduct follow-ups and evaluation visits in designated territory.
- Make home visits to patients in specified area.
- Reconcile medications and perform wound care and other procedures.
- Monitor patients' conditions and report changes to the clinical team.
- Collaborate with physicians and other staff to ensure quality care.
- Document observations, interventions and evaluations of patient care.
- Initiate and administer IV antibiotics and diuretics per protocol.
