Job summary

Moments Hospice is seeking a dedicated Registered Nurse to join our on-the-road care team. You will perform initial assessments, refine care plans, and deliver hands-on nursing care with a focus on patient education. Strong collaboration with families and the healthcare team is essential.

Travel requirements involve coordinating patient care across multiple settings with a vehicle provided by the company. Proficiency with EMR and excellent planning and communication skills are required to ensure

Qualifications

• Must be licensed RN in the state of employment.
• Proficient with Electronic Medical Records (EMR) software.
• Strong planning, organizing and communication skills.
• Ability to work autonomously while meeting responsibilities.
• Valid Driver's License and reliable vehicle with current insurance.
