Job summary

Moments Hospice in Wisconsin is seeking a Registered Nurse to provide hands-on nursing care, patient education, and coordinated care for an on-the-road caseload. You will conduct initial assessments and refine care plans in collaboration with families and the healthcare team.

Qualifications

• Must be Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.
