Job summary

Moments Hospice is seeking a Registered Nurse to conduct initial client assessments and continually refine care plans while delivering hands-on nursing care and patient education. You will maintain open communication with patients, families, and the healthcare team, and manage care coordination for your caseload.

Qualifications

• Licensed RN in Minnesota is required.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Valid Driver's License and reliable vehicle with current insurance.
