Job summary

Moments Hospice is seeking a Registered Nurse to provide high-quality patient care on the road. The role includes hands-on nursing care, patient education, and coordinating care across a multidisciplinary team.

You will assess clients, develop and refine care plans, communicate with families and healthcare professionals, and ensure smooth care coordination for your caseload. A driver’s license and reliable vehicle are required.

Qualifications

• Licensed RN in Wisconsin.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and healthcare team.

• Valid Driver's License and reliable vehicle with current insurance
