Job summary

Moments Hospice is seeking an on-the-road Registered Nurse to provide hands-on care, conduct client assessments, and coordinate care across the healthcare team. You’ll travel to patient sites, educate families, and ensure high-quality nursing services.

Qualifications

• Licensed as a Registered Nurse (RN) in the state you work in.

• Ability to solve problems autonomously while maintaining high standards.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings for comprehensive care planning.
