Job summary

Moments Hospice is seeking a Registered Nurse to provide hands-on nursing care and patient education on the road. The role emphasizes care coordination, engaging with patients, families, and the healthcare team to continually refine care plans.

We require an active RN license, EMR proficiency, and strong planning and communication skills. A valid driver's license and reliable vehicle with insurance are essential for this travel-based position.

Qualifications

• Active RN license in the state you work in.
• Proficient with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
