Job summary

Moments Hospice is seeking an on-the-road Registered Nurse to conduct client assessments, deliver hands-on nursing care, and coordinate home-based care plans. You will educate patients and families while collaborating with the care team to ensure continuity of services.

The role emphasizes autonomous problem-solving, strong organization, and effective communication, with a comprehensive benefits package and a company car provided for both business and personal use.

Qualifications

• Licensed RN in the state you work in.
• Proficient with EMR software.
• Strong planning, organizing and communication skills.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families, and the healthcare team.
• Coordinate patient care for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
