Job summary

Moments Hospice is seeking a dedicated Registered Nurse to join our on-the-road care team. You will perform initial assessments, deliver nursing care and patient education, and coordinate care with families and the healthcare team.

Competitive salary ($75,000–$90,000/year) and a comprehensive benefits package including a company car with fuel and insurance, health/dental/vision coverage, generous PTO, and a 401(k) with company matching.

Qualifications

• Licensed RN in the applicable state.
• Proficiency with EMR software.
• Strong planning, organizing and communication skills.
• Ability to solve problems autonomously while meeting standards.
• Valid driver's license and access to a reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families and the healthcare team.
• Coordinate patient care for your caseload.
• Participate in interdisciplinary team meetings for comprehensive planning.

• Provide hands‑on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Qualifications:

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance
