Job summary

LiveWell Homecare Agency, LLC seeks an experienced RN Case Manager Supervisor to oversee the NV Medical Management Command Center, managing discharge planning, transitions of care, and utilization oversight for long-term hospitalized members. You will lead IDT meetings, drive performance metrics, and mentor staff to ensure compliance and excellent service.

The role requires Nevada RN licensure, CCM certification within 2+ years, and 1+ year in managed care with supervisory experience.

Qualifications

• Active Nevada RN license.
• CCM certification within 2+ years.
• 1+ year of managed care and supervisory experience.
• Strong clinical, regulatory and technology knowledge.
• Bachelor’s degree is preferred.

Responsibilities

• Oversee RN case management functions including discharge planning, transitions of care and utilization management for long-term hospitalized members.
• Lead IDT meetings and participate in taskforces.
• Manage performance metrics and reporting to leadership.
• Develop team through training and coaching; support to meet compliance and customer service standards.
• Promote cross-department collaboration and apply LEAN principles for performance improvement.
