Job summary

Qualifications

• CT RN license required.
• ASN required, BSN preferred.
• Two years acute-care clinical nursing experience required; case management, home care, or managed care experience strongly considered.
• Excellent communication, negotiation and conflict resolution skills required.
• Knowledge of computer applications and reimbursement systems preferred.

Responsibilities

• Facilitate patient throughput for assigned inpatient population.
• Assess patients for appropriate level of care and coordinate care and services across the continuum.
• Identify risk factors and coordinate involvement of support services.
• Develop and implement transition planning activities and document in case management systems.
