Job summary

Hartford Hospital is seeking a Case Coordinator to facilitate patient throughput for an inpatient population. You will assess patients for appropriate level of care, coordinate services across the continuum, identify risk factors, and collaborate with the healthcare team to implement transition planning.

You will document care in our case management systems and contribute to process and quality improvement initiatives.

Qualifications

• CT RN license required.
• ASN required; BSN preferred.
• Two years acute-care nursing experience; case management/home care/manged care experience considered.

Responsibilities

• Assess patients for appropriate level of care and coordinate their care across the continuum.
• Identify risk factors and facilitate involvement of support services.
• Perform utilization management and develop transition planning activities.
• Document aspects of care in case management systems and collaborate with the health care team on quality improvement initiatives.
