Job summary

Hartford Hospital, a major teaching hospital and Level 1 Trauma Center in Connecticut, seeks a Case Coordinator to optimize patient throughput and transition planning. You will assess care needs, coordinate across disciplines, and document in case management systems while collaborating with a multidisciplinary team.

This role emphasizes inpatient population management, utilization activities, and a commitment to high-quality patient advocacy within a leading health network.

Qualifications

• CT RN license is required.
• ASN is required, BSN preferred.
• Two years acute-care nursing experience; case management, home care, and/or managed care experience is a plus.
• Excellent communication, negotiation and conflict resolution skills are required.

Responsibilities

• Facilitate patient throughput and coordinate care across the continuum.
• Assess patients for appropriate level of care and identify risk factors.
• Coordinate services and involve support teams for transitions.
• Perform utilization management and document in case management systems.
• Develop and implement transition planning activities.
• Collaborate with health care team to improve process and quality.
