Qualifications

• Certification in case management desired.

Responsibilities

• Facilitate patient throughput for assigned inpatient population.
• Assess patients for appropriate level of care and coordinate across the care continuum.
• Identify risk factors and involve support services as needed.
• Perform utilization management and document in case management systems.
• Collaborate with healthcare team on process and quality improvement.
