Job summary

Hartford Hospital is seeking a Case Coordinator to support patient throughput and discharge planning for an inpatient population. The role requires coordination across the care continuum, assessment of care levels, and collaboration with the healthcare team to ensure timely transitions.

The successful candidate will hold a CT RN license with 2+ years of acute-care experience. Strong communication and problem-solving skills are essential, with knowledge of reimbursement systems and case

Qualifications

CT RN license required.
ASN required; BSN preferred.
Two years acute-care and diversified clinical nursing experience, including medical/surgical.
Case management, home care, and/or managed care experience strongly considered.
Excellent communication, negotiation and conflict resolution skills required.
Knowledge of computer applications and reimbursement systems preferred.
Certification in case management desired.

Responsibilities

Facilitate patient throughput and assess patients for appropriate level of care.
Coordinate care and services across the care continuum.
Identify risk factors and involve support services as needed.
Perform utilization management and develop transition planning activities.
Document care in case management applications and systems.
Collaborate with health care team members on process and quality improvements.
