Job summary

Anna Jaques Hospital seeks an experienced RN Case Manager to partner with the interdisciplinary care team in the Triad Model of Care Transitions. The role drives efficient disposition planning, coordinating care transitions and ensuring safe, timely discharges for hospitalized patients.

The RN Case Manager will navigate health system services, advise on healthcare compliance, and influence throughput, clinical effectiveness, and fiscal responsibility within a dynamic inpatient setting.

Qualifications

• Massachusetts RN licensure required.
• Bachelor’s degree in nursing or healthcare-related field preferred.
• 3–5 years of experience in an acute care setting preferred.
• ACM, CCM, or CMAC certifications preferred.
• BLS certification required.

Responsibilities

• Coordinate care transitions and discharge planning for hospitalized patients.
• Collaborate with interdisciplinary team to optimize patient flow and outcomes.
• Document care plans, progress notes, and transition needs clearly and timely.
• Identify barriers to care and work with payer to verify coverage and benefits.
• Participate in multi-disciplinary rounds and contribute to quality improvement.
