Job summary

Independence Health System Inc. is seeking a Clinical RN Case Manager to coordinate patient care, plan discharges, and support transitions across the care continuum. You will collaborate across ED, inpatient, and community partners to promote outcomes and reduce readmissions.

You will provide leadership within the team, maintain compliant documentation, and ensure alignment with payor requirements while supporting patients and families through complex healthcare decisions.

Qualifications

• Graduated of an accredited nursing program.
• Two (2) years recent healthcare/clinical experience, or recent clinical experience in an emergency department, case management, or utilization review position.
• Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
• Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
• Knowledge of payor/insurance benefits
• Functional Skills on PC and Related Software (Microsoft Office)
• Knowledge of basic office equipment such as copier, fax machine, etc.
• Ability to understand and react effectively in a fast-paced environment. Must have flexibility/prioritization skills.

Responsibilities

• Assesses, plans, coordinates, and monitors and evaluates options for patients and families to promote care coordination outcomes.
• Manages transitions of care effectively to reduce readmissions and poor health outcomes.
• Initiates evidence-based care coordination strategies.
• Establishes compassionate environment and supports patients, families, and care givers.
• Promotes patient independence through education on condition, medications, and self-care.
• Maintains documentation and communications to support care plans and transitions.
• Collaborates with multidisciplinary teams to progress the plan of care.
• Ensures payor requirements are understood and communicated to patients or representatives.
