Job summary

UCI Health is seeking a skilled RN Case Manager to lead high-level assessment, care planning, and interdisciplinary coordination across the health system. You will advocate for patient self-determination, ensure safe transitions of care, and maintain accurate clinical documentation with a focus on quality outcomes.

Qualified candidates have a BSN, 5+ years of clinical experience with at least 1 year in case management, and strong communication skills.

Qualifications

• Minimum 5 years of clinical experience and 1 year of Case Management experience.
• Current RN License.
• All external candidates must have a Bachelor of Science in Nursing.
• Excellent written and verbal communication skills in English.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.

Responsibilities

• Perform high-level assessment and reassessment to plan and implement goal-directed care.
• Collaborate with interdisciplinary team to coordinate high-quality patient care and transitions.
• Facilitate interdisciplinary rounds and advocate for patient self-determination and choice.
• Monitor resource utilization and ensure accurate clinical documentation.
