Job summary

Johns Hopkins Intrastaff is seeking a Nurse Case Manager to coordinate patient-centered care across the continuum from pre-admission through post-discharge. You will lead the transition planning with the care team and collaborate with physicians, social workers and payors to optimize outcomes.

Ideal candidates will have a BSN (MSN preferred), Maryland RN license, and 5–7 years of nursing experience with at least 1 year in case management or discharge planning.

Qualifications

• RN with valid Maryland Nursing License.
• Completion of an accredited BSN Nursing Program; MSN preferred.

Responsibilities

• Coordinate patient-centered care across the continuum from pre-admission to post-discharge.
• Identify/readmission risks and plan transition needs with patient/family and care team.
• Lead multidisciplinary rounds and collaborate with physicians, payors and post-acute providers.
• Educate patients/families to optimize self-care and independence.
