Job summary

Northeast Georgia Health System is seeking an experienced RN Case Manager to oversee assessment, planning, implementation and evaluation of patient care across age groups. The role collaborates with physicians, nursing staff and families to ensure efficient, quality, cost-effective outcomes and coordinated discharge planning.

Responsibilities include managing length of stay, regulatory compliance, and participating in interdisciplinary rounds.

Qualifications

• Licensed to practice as an RN in Georgia.
• Associates Degree; graduate of an accredited school of nursing.
• Minimum 3–5 years of direct patient care and/or case management experience.

Responsibilities

• Assess and plan patient care across populations (neonate to geriatric).
• Coordinate with physicians, patients, families and care team for safe discharge planning.
• Monitor length of stay and ensure regulatory compliance.
• Facilitate transitions to next level of care and post-hospital needs.
