Job summary

Mission Hospital in Asheville, NC, part of HCA Healthcare, seeks a Case Manager to support patients and families through every step of care, coordinating services, developing plans, and partnering with physicians, nurses, and leaders to ensure safe transitions and consistent quality.

The requirements include an RN with an Associates degree, BLS, and 2+ years in case management (or 3+ years in a clinical setting); InterQual experience preferred.

Qualifications

• 2+ years experience in Case Management OR 3+ years experience in a clinical setting.
• InterQual experience preferred.
• Associates Degree in Nursing required.
• Masters Degree in Nursing preferred.
• Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
• BLS certification required.

Responsibilities

• Provide case management services for inpatient and observation patients as assigned.
• Identify patients at risk for adverse outcomes during transitions between care settings.
• Perform a comprehensive psychosocial, medical and discharge needs assessment.
• Coordinate the plan of care and drive the discharge plan with the multidisciplinary team.
• Partner with Social Services to ensure post-acute medical needs are met.
• Evaluate progression of care using InterQual criteria and escalate as needed.
• Liaise effectively with physicians, patients, families, hospital staff and external agencies.
