Job summary

Qualifications

Responsibilities

• Provides continuity of care across care settings to ensure smooth transitions.
• Builds relationships with patients and multidisciplinary teams to support treatment and follow-up.
• Manages high-risk, complex patient care to minimize readmission.
• Coordinates care plans with physicians and team members and communicates with the patient.
