Job summary

Ochsner Health in Louisiana seeks a nurse liaison and discharge coordination specialist to work with patients, caregivers, and a multi-disciplinary team to ensure smooth transitions across care settings. The role requires an RN with at least 2 years of nursing experience, current licensure, and BLS.

You will collaborate with physicians to maintain a coordinated care plan and advocate for patients. This on-site position offers opportunities to impact patient outcomes while navigating complex care

Qualifications

• Graduate of an accredited school of nursing.
• Preferred - Associate's or Bachelor’s degree in nursing.
• Required - 2 years of nursing experience.
• Current RN license in state of practice.
• Basic Life Support (BLS) from the American Heart Association.
• Preferred - Certification in clinical specialty area.

Responsibilities

• Provides continuity of care across settings.
• Builds relationships with patients and multidisciplinary team for follow-up.
• Manages high-risk, complex patients to minimize readmission.
• Coordinates care plan with physicians and team; advocates for patient.
• Prepares and reinforces post-discharge plan.
• Identifies barriers to care and drives process improvements.
• Collaborates with leadership to improve patient experience on referrals.
• Adapts communication to patient population and privacy needs.
• Performs other related duties as required.
