Ochsner Health in New Orleans, LA seeks an experienced Case Manager RN to oversee complex patient care across the continuum. You will assess needs, develop and coordinate treatment plans with the PCP team and providers, and monitor outcomes to optimize care and reduce hospitalizations.

Responsibilities include identifying social determinants, coordinating with social work, and ensuring compliance with CMSA standards.

Qualifications

• RN license in state of practice.
• Graduate of accredited school of nursing.
• Minimum 3 years of experience in a clinical setting.
• Experience documenting in an electronic medical record.
• Experience working in a multidisciplinary team environment.
• CCM certification (preferred).

Responsabilités

• Develop and implement coordinated treatment plans across the continuum.
• Assess social determinants of health and address barriers to care.
• Coordinate referrals and appointments with the care team.
• Provide in-depth patient education and collaborative self-management plans.
• Identify quality issues and report to leadership.
