Option Care Health, Inc. in Detroit, MI is seeking a Clinical Care Transition Specialist (RN) to educate discharge patients and families on home infusion services.

You will work with hospital staff to ensure smooth transitions to home or alternative sites and support timely therapy initiation. Licensed RN (or pharmacist) with 2+ years in healthcare uses consultative sales to promote services, coordinate orders, and collect pre-admission information.

Qualifications

• Experience in healthcare with ability to educate patients and families.
• Ability to build relationships with hospital staff, case managers and discharge planners.
• Proficient in using MS Word, Excel, PowerPoint and basic computer skills.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with hospital staff to sell Option Care services and assess candidate patients.
• Educates patients and families about home infusion services and therapy initiation.
• Coordinates pre-admission information and secures related signoffs.
• Partners with Account Manager to execute area business and growth plans.
• Ensures confidentiality of patient information and complies with legal guidelines.
