Job summary

Option Care Health, Inc. in Hayward, CA is seeking a Clinical Transition Specialist (RN or Pharmacist) to educate patients and families, coordinate discharges, and grow referrals for home infusion services.

You will work with hospital staff and internal teams to ensure timely, quality care at an alternate site. Applicants should have an active RN or Pharmacist license, 2 years in healthcare, and strong relationship-building/communication.

Qualifications

• Licensed RN or Pharmacist in the state of practice.
• Minimum 2 years in healthcare.
• Strong relationship-building with referrals and hospitals.

Responsibilities

• Maintain and grow relationships with referral sources to increase sales and patient starts.
• Educate patients, families and facility staff about services and products.
• Coordinate discharge planning and timelines with hospital and internal teams.
• Partner with Account Manager to develop area business and growth plans.
• Ensure accurate pre-admission information and signoffs.
