Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate discharging patients and their families about home infusion services. This role focuses on placing patients in appropriate home/alternate site settings and coordinating care transitions.

Build strong referral relationships to grow service referrals and work with Account Managers to meet sales goals. Requires RN or Pharmacist license and 2+ years in healthcare, with strong communication and presentation skills.

Qualifications

• Licensed RN or licensed Pharmacist in the state of practice.
• At least 2 years in healthcare.
• Experience building relationships with internal/external stakeholders.
• Proficient PC skills: Word/Excel/PowerPoint.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell Option Care services and assess candidate suitability.
• Educates patients, families, and facility staff about services and products.
• Provides in-person education to patients and caregivers for timely discharge and independence.
• Communicates with agency staff, medical team, patients, and family throughout discharge to implement treatment plans.
• Collects and reviews pre-admission information and secures signoffs.
• Partners with Account Manager to create and execute area growth plans.
• Collaborates with marketing to deliver educational and promotional programs.
