Job summary

Option Care Health is seeking a Clinical Transition Specialist to drive patient transitions from hospital to home infusion settings. This role focuses on educating patients and families, managing referrals, and coordinating with discharge planners.

Applicants must be a licensed RN or Pharmacist with healthcare experience and a proven ability to build relationships with providers and care teams. Travel to care facilities is required.

Qualifications

• Licensed RN or Pharmacist in the state of practice with healthcare experience.
• Experience establishing relationships with healthcare providers and vendors.
• Strong customer service and communication skills with internal/external stakeholders.

Responsibilities

• Grow referrals and educate discharging patients about home infusion services.
• Serve as a primary contact for referral sources and Account Managers.
• Assess patient suitability for alternative site care and coordinate transitions to home/ATS.
• Collaborate with intake, case managers and discharge planners to ensure quality service.
• Coordinate education for patients, families, and facility staff to enable timely discharge.
